This dataset provides a current 1 to 1 to 1 healthcare facility crosswalk between the Department of Health Care Access and Information (HCAI) assigned license numbers and the California Department of Public Health (CDPH), Licensing and Certification facility lists based on the assigned license number housed within the Electronic Licensing Management System (ELMS) and/or the CMS Certification Number (CCN) from the Federal Automated Survey Process Environment (ASPEN) databases. Data source is updated quarterly.

CCN | text | CCN (CMS Certification Number) is a 6-digit facility identifier used by CMS to verify that a provider has been Medicare certified and for what type of services. In order to avoid confusion with the National Provider Identifier (NPI), the Medicare/Medicaid Provider Number (also known as the OSCAR Provider Number, Medicare Identification Number or Provider Number) had been renamed the CMS Certification Number (CCN). The first two digits are the numeric state code. NOTE: The CCN for ambulatory surgical centers is a 10-digit alphanumeric combination (######, ##A####, ##C#######). Note that nine of the facilities operate under the certification of a “parent” facility. Facilities in the same parent-child groups operate under the same CMS Certification Number (CCN), and the “child” facility’s surveys are attributed to the parent facility (based on the federal facility ID). |

NPI | text | "National Provider Indicator (NPI) is a 10-digit identifier for health care providers and health plans that was mandated by the Health Insurance Portability and Accountability Act of 1996 (HIPPA) for use in electronic transactions. Individuals or organizations apply for NPIs through the CMS National Plan and Provider Enumeration System (NPPES). Individual NPIs, also known as Type 1 NPIs, are for healthcare providers, such as dentists. Organizational, or Type 2 NPIs are for use by incorporated businesses, such group practices and clinics.
